SCC Program Coordinator
Full-Time Position Available

Position Summary:

The Supporting Community Connections (SCC) Program Coordinator supervises, coordinates, and monitors all program components to ensure adherence to Children’s Receiving Home policies and procedures. Provides direct supervision to the Youth Outreach Worker. Directs all employees contributing to and associated with the program. Coordinates all program activities and outreach events. Fosters relationships with community partners, agencies, and organizations. Provides culturally relevant outreach, engagement, and support services to transition aged youth and those who work/live with them, designed to promote support services. They also provide advocacy to program participants and coordination with the community and collaborative partners. This worker is an individual with experience as a “system” youth, or a family member of a “system” youth, and/or be culturally competent in the sub-cultures of LGBTQ youth, homeless youth, and/or foster youth. He/she will provide supportive services in the community as well as at the CRH offices. The worker will also participate in community outreach and education activities and co-facilitate psycho-educational, social, and other groups as needs are identified.

Essential Duties and Responsibilities:

  • Provide culturally relevant outreach activities designed to promote support services to youth and TAY.
  • Provide or link suicide prevention support services to individuals feeling disconnected or at risk for suicide
  • Become updated on community resources
  • Provide groups for youth seeking their input, advice, and involvement
  • Seek youth input on further design of this program
  • Initiate, organize, and/or participate in community events and educational opportunities related to wellness and prevention services.
  • Provide cultural brokerage, and advocacy for individuals and families to facilitate accurate understanding of the participant risk and protective factors and appropriate service planning and access.
  • Identify and develop community resources and natural support systems for program participants and families. Make referrals, assist with service linkage, and educate individuals and families on how to access and use community resources and services.
  • Participate in community meetings as a representative of the agency. Act as a liaison for the agency to various other community agencies and organizations.
  • Recruit community members to participate in wellness and prevention activities.
  • Attend and participate in trainings and meetings as required.
  • Complete required documentation in an accurate manner, and within required timelines
  • Write and submit Quarterly Reports to the Sacramento County contract monitor
  • Serve as an advocate for youth at all times
  • Provides direct supervision to the Youth Outreach Worker
  • Perform other related work as assigned.


  • Possession of a Bachelor’s Degree from an accredited college or university is required.
  • Experience of as “system” youth, or family member of a “system” youth required.
  • Must have cultural expertise with TAY sub-cultures including LGBTQ, homeless, and foster youth.
