TSCF social workers are assigned to provide coordination of
care for children and youth. Working in close proximity to
clients allows our social workers to identify individual
strengths and needs for each youth. When children and youth
arrive to the TSCF, social workers provide an assessment of
client needs and links to community resources.
Children and youth receive coordination of care for school,
medical appointments and mental-health services and
referrals. Social workers have strong working relationships with
the child’s Child Protective Services social workers. When
children and youth are ready to transition, our social workers
coordinate pre-placement visits with potential foster parents or
residential treatment programs.
